We like to use this model as a survey to get a baseline measurement of a team’s current state. We do this by asking team members to (anonymously) rate where they see the team within each area on a scale of 1-7 where ‘1’ is ‘Not at all’ and ‘4’ is ‘So-so’ and ‘7’ is ‘Extremely.’ It is interesting to see an average of the scores and also where there are large gaps in the perceptions among the team members. The survey results are a starting place for discussions about what actions to take to improve the performance of the team. Improvements can be evaluated based on subsequent surveys of the team members.
